AUTHORIZATION TO RELEASE PHOTOGRAPH, FILM OR VIDEOTAPE
I hereby give Workforce Solutions (including any of its officers, employees, contractors and agents) absolute and irrevocable right and permission with respect to the photographs/film/ video to be taken of me, or in which I may be included with others:
• To copyright the same in his/her own or company name.
• To use, re-use, publish and re-publish the same in whole or in part, individually or in conjunction with other photographs/film/videos, in any medium and for any purpose whatsoever, including (but not by way of limitation) illustration, promotion, advertising and trade.
• To use my name in connection therewith, if he/she or company so chooses.
I understand that I will not be given any monetary compensation for my time or services.
I hereby release and discharge Workforce Solutions (including any of its officers, employees, (contractors and agents) from all claims and demands arising out of, or in connection with, the use of the photographs/film/video, including any and all claims for libel.
